To schedule your DNA sample collection at KaturaLab (Cincinnati, OH), you must follow these instructions:
1. Contact DNA Testing Lab of Choice Request that your sample be collected at KaturaLab LLC. We do not accept direct scheduling.
Your testing lab will:
- Confirm your case details
- Schedule your appointment with KaturaLab
- Send the official chain of custody paperwork and DNA collection kit to our site.
3. Complete Chain of Custody Intake Form Complete this intake form and email is to us at info @ katuralabs.com
Do not sign the document until your collection appointment in the presence of the technician.
4. Complete Payment

- Chain of Custody Documentation $50.00
5. Prepare for Collection Wash your hands thoroughly. Avoid eating, drinking, smoking, or chewing gum for at least 30 minutes before swabbing.
